Proses pengelolaan rantai pasok di Perkebunan Karet Pak Yasin hingga saat ini masih dilakukan secara manual tanpa dukungan sistem berbasis data yang terintegrasi. Permasalahan utama yang diidentifikasi meliputi tidak adanya sistem perencanaan pengadaan bahan pendukung, tidak terjadwalnya proses panen, serta kurangnya pengelolaan distribusi yang terintegrasi. menyebabkan penurunan efisiensi dan potensi kehilangan kualitas produk. Penelitian ini bertujuan untuk merancang dan membangun sistem informasi berbasis web yang dapat mendukung pengelolaan rantai pasok secara lebih terstruktur. Penelitian ini menggunakan pendekatan deskriptif kualitatif dengan metode observasi, wawancara, dan studi pustaka. Model SCOR diterapkan untuk memetakan proses bisnis mulai dari Plan, Source, Make, Deliver, hingga Return, Sebagai solusi, dirancang sistem berbasis web yang mendukung perencanaan pengadaan, penjadwalan panen, serta pengelolaan distribusi dan kontrol kualitas. Sistem ini diharapkan mampu meningkatkan efisiensi operasional, akurasi data, dan ketepatan waktu dalam setiap proses rantai pasok. Penelitian ini juga memberikan referensi praktis dalam penerapan SCM di sektor perkebunan rakyat berbasis digital.

The supply chain management process at Pak Yasin Rubber Plantation is still conducted manually without the support of an integrated data-based system. The main problems identified include the absence of a system for planning the procurement of supporting materials, unscheduled harvesting processes, and a lack of integrated distribution management. These issues lead to decreased efficiency and potential loss of product quality. This study aims to design and develop a web-based information system to support a more structured supply chain management process. A descriptive qualitative approach was used, employing observation, interviews, and literature studies. The SCOR (Supply Chain Operations Reference) model was applied to map the business processes from Plan, Source, Make, Deliver, to Return. As a solution, a web-based system was designed to support procurement planning, harvest scheduling, distribution management, and quality control. The system is expected to improve operational efficiency, data accuracy, and timeliness in each supply chain process. This research also provides a practical reference for implementing SCM in smallholder plantation sectors through digital solutions.
